If you have had recent blood work or other diagnostic tests done, please bring a copy of those results with you. If you have any questions, please feel free to reach out to us.
For patients with insurance, please use the insurance verification form on the INSURANCE page to submit your insurance information. We recommend that you provide us with your insurance information at least two days prior to your appointment so that we can verify your coverage. We will contact you with details regarding your acupuncture coverage and benefits.

If you need to cancel an appointment for any reason, please contact us and let us know as soon as possible.
PAYMENT
Payment in full is expected at the time of service. For insurance patients, co-pays and deductibles are due at the time of service. Cash, personal checks, HSA cards, and major credit cards (Visa, Mastercard, Discover, American Express) are accepted forms of payment. All returned checks will be charged a $30 fee along with the amount that you originally owe.
